TV6 Issue: Muted Player Calls, Voice Commands, say/teamsay

Several players are having a problem using voice commands, calling for medic or even typing team or general chat messages on TV6 DUSTBOWL
I have used 2 systems, Windows 7 x64 at home and Linux Mint elsewhere and am continuing to have the problem.
I was told that restarting steam would resolve it, it did not. Reinstalling TF2 did not but I did re-include my player scripts and custom hud but I wouldn't think it would do anything. Here's everything I think would be relavent:
Windows 7 Ultimate SP1
Processor: Intel(R) Core(TM) i7-2600 CPU @ 3.40GHz
Package Socket 1155 LGA (0x1)
Core Speed 3392.8 MHz
Chipset: Q65
Memory: DDR3 16gig 665.5MHz xFSB Multi.
Video: AMD Radeon HD 7700 Series
Troubleshooting:
- Restarted TF2
- Restarted Steam
- Restarted System
- Deleted Local Cache, re-installed/downloaded TF2
I have been having another issue every time I close TF2 it "crashes" causing a general system error (image : http://i.imgur.com/WwQGZS2.png)

No help there either it would seem.
Considering this is only happening to a few people and most seem to resolve it with a restart there's something wrong with my (and a couple of other people's) configuration.
If you had this issue and found resolution please help
